LiveWell
LiveWell is an optional online service provided by Advocate Aurora. LiveWell helps you, as a patient of Advocate Aurora, to access portions of your health information and to communicate with your Advocate Aurora healthcare providers.
LiveWell is not intended for urgent communication. If you are in need of urgent medical attention please call your healthcare provider’s office or call 911 in case of emergency.
You agree to use LiveWell appropriately and solely for the purposes and with the limitations stated in these Terms of Use. LiveWell is provided for your convenience and its availability is not an entitlement, right, or privilege. Advocate Aurora may limit or remove your access to LiveWell at any time.
Purpose of LiveWell
The purpose of LiveWell is to provide you with (1) access to your medical records, such as basic laboratory results, summary medical history, visit history, and billing information, and (2) the ability to communicate with your Advocate Aurora healthcare provider. Some information accessed through LiveWell may assist in providing medical, nursing, or other professional services or advice. However, the information provided in LiveWell is not a substitute for medical advice, consultation, diagnosis, or treatment.
Use/access of LiveWell
All information in LiveWell is provided “as is” without warranty of any kind and meant for use only to support your relationship with your healthcare provider. You understand that not all of your Advocate Aurora medical record may appear in your LiveWell account. For records that are not immediately accessible on your LiveWell account, you can request these records by accessing the “Request Medical Records” icon from your LiveWell account. In addition to your LiveWell account, Advocate Aurora Health has a web page for Illinois and Wisconsin that list several other options to request your medical records. If you are in Illinois, please log on to https://www.advocatehealth.com/contact-us/medical-records, or if you are in Wisconsin, please log on to https://www.aurorahealthcare.org/patients-visitors/medical-records to find the correct Health Information department phone numbers and forms as needed.
Advocate Aurora will make its every reasonable effort to provide a timely response to your electronic messages. In some situations, the clinic staff that needs to respond to a message may not be immediately available. Therefore, the anticipated turnaround time for a response to a patient inquiry is two business days at a minimum.
The information you submit to LiveWell, including your electronic messages, may be shared with Advocate Aurora staff when their job includes assisting your Advocate Aurora healthcare provider in providing your medical care. However, your information will only be available to those staff members who have a business reason for accessing it. If your provider is out of the office or unavailable, messages sent within LiveWell may be routed to other appropriate and authorized caregivers within Advocate Aurora in order to facilitate a timely response. In addition, Advocate Aurora is only able to respond to electronic messages based on the information provided by you. Therefore, if there is insufficient information provided, Advocate Aurora may be unable to provide accurate and reliable responses and/or service.
LiveWell may offer links to medical websites that are not a part of Advocate Aurora. These website links are provided for general information purposes only and we do not endorse and have not verified the accuracy of the information on these websites. You should not rely on the information for purposes of treatment or diagnosis.
You may choose to use other services provided by third parties in conjunction with your use of LiveWell. Such services may include but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Advocate Aurora has no control over the contents of these services and does not accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of LiveWell, you do so entirely at your own risk and subject to the terms and conditions of use for such services.
LiveWell: privacy
The privacy of your health information is very important to us. The information in LiveWell may contain sensitive information and you should take precautions to protect the confidentiality of the information. Advocate Aurora may use or share your health information as described by the Advocate Health Care Notice of Privacy Practices and the Aurora Health Care Notice of Privacy Practices. Advocate Aurora will maintain the privacy of your health information and will not disclose it to any third parties without your authorization or as otherwise permitted by law. Please review the Advocate Health Care Notice of Privacy Practices and the Aurora Health Care Notice of Privacy Practices carefully for an explanation of how, when, and why we use and disclose your health information.
LiveWell: proxy access
If you are the parent or legal guardian of a child up to the child’s 18th birthday or the legal guardian or holder of healthcare power of attorney for individuals age 18 or older, you may request access to that individual’s LiveWell account “Proxy Access”. You will need to sign the appropriate Proxy Access form and must inform Advocate Aurora immediately if your relationship with the account holder changes.
Adolescent proxy access (ages 12-17)
Please be mindful that the confidentiality of adolescents’ health information is protected by a combination of state and federal laws. Though laws vary by state, in each state adolescent minors have the right to consent to certain health care services; and some adolescent minors are allowed to consent for all or most of their own care. Accordingly, to comply with these laws, for patient ages 12-17 not all records are made immediately accessible via proxy access. If you wish to obtain additional records for your minor child, Advocate Aurora Health has a web page for Illinois and Wisconsin that list several other options to request medical records. If you are in Illinois, please log on to https://www.advocatehealth.com/contact-us/medical-records, or if you are in Wisconsin, please log on to https://www.aurorahealthcare.org/patients-visitors/medical-records to find the correct Health Information department phone numbers and forms as needed.
LiveWell: e-mail communications
You may receive e-mail messages to your e-mail account advising you that new information has been added to your LiveWell account. Please be advised that email is not secure and there is a risk that these emails could be read by a third party, including anybody who has access to your email account, such as your spouse. Although we will not send any private medical information via email, the notification that new medical information is available by accessing LiveWell may be information that you would not want others to know. Thus, you should take this into account when providing an e-mail address or using LiveWell. By signing up for LiveWell, you acknowledge the inherent risks of email communications and consent to receiving e-mail messages regarding your LiveWell account.
Security and confidentiality
Advocate Aurora affords the same degree of confidentiality to medical information stored in LiveWell as is given to medical information stored by us in any other medium. We are committed to protecting the confidentiality of your medical information. We limit our employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We will identify the records released and note the time and date of access each time you access LiveWell. We have taken steps that are designed to make all information received from our LiveWell subscribers as secure as possible and protect against unauthorized access and use.
For other than general information viewing, LiveWell must be accessed with a Secure Sockets Layer (SSL) compatible browser. Usernames and passwords provide two layers of authentication. As a LiveWell user, your role in maintaining the security of your medical information is to: 1) Change your password on a regular basis, and 2) Keep your login ID and password confidential. If you share your LiveWell ID and password with another person, that person may be able to view your health information, and health information about someone, like your child, for whom you have Proxy Access. It is your responsibility to select a confidential password, to maintain your password in a secure manner, and to change your password if you believe it may have been compromised in any way.
Disclaimer
You understand and agree that the use of LiveWell is voluntary and entirely at your own discretion and that of Advocate Aurora.
Advocate Aurora will not be liable for any direct, indirect, consequential, special, exemplary, punitive or other monetary damages, fees, fines, penalties or liabilities arising out of or relating to this service. You agree to defend, indemnify and hold Advocate Aurora harmless from and against any claims, actions, judgments, and settlements, including legal fees that arise from your misuse of LiveWell.
Discontinuing use of LiveWell
You are not required to use LiveWell and you can deactivate your LiveWell account at any time by contacting our customer support at 855-624-9366.
Classes and events
You may sign up for email notification for classes and events offered by Advocate Aurora. You may also register online for available classes and events. Any payments for classes and events may be made online using a credit or other payment card. Additional information on credit or other payment card payments is set forth below. If you sign up for email notification or otherwise register for a class or event, we will use the information you provide us as set forth in the Privacy Policy.
Credit card and payment card information
If you are a registered participant in LiveWell, you may make credit card payments via LiveWell for services provided by Advocate Aurora. Charges to you for services provided by Advocate Aurora are subject to the limitations of your individual insurance coverage. Please always check with your insurance carrier regarding benefit coverage.
You may also submit credit card or payment card information via this Website for payment of registrations for classes and events and online donations. You must submit the necessary card information before your payment or order will be processed. If the card information that you submit is incorrect or invalid, your payment or order will not be processed. Payment is subject to the approval of the financial institution issuing the card. We assume no responsibility or liability if the financial institution refuses to accept or honor your card for any reason.
Online employment applications
Advocate Aurora accepts applications for employment online. You may register to submit your application or resume online, subject to additional terms and conditions applicable to such online employment application process. Submission of your resume or online application for employment does not guarantee or entitle you to any employment or consideration for any particular position.
Restrictions on your use
You may view material from the Website for your own personal use or the internal use of your business. You may not download, reproduce or otherwise distribute any materials on this Website without our prior express written consent and/or the consent of any third party copyright owner.
Any rights that are not expressly granted by these Terms of Use or any applicable end-user license agreements are reserved by Advocate Aurora.

If your provider is located in Wisconsin, you submit to the exclusive jurisdiction of the state and federal courts sitting in the County of Milwaukee in the State of Wisconsin and waive any jurisdictional, venue, or inconvenient forum objections to such courts. In any action to enforce these Terms of Use, Advocate Aurora will be entitled to all costs and attorney fees it incurs.
Any cause of action or claim you have regarding the Website or the LiveWell service must be commenced within one year after the claim or cause of action arises or the claim or cause of action is barred.
Notice may be given to you as a user of this site by posting information to this site. Notice may be given to Advocate Aurora only by delivery to Aurora Public Relations Department, 750 West Virginia Street, P.O. Entire agreement
These Terms of Use, along with the Privacy Policy, constitute the entire agreement between Advocate Aurora and you pertaining to the Website.
