We may use third party service providers to assist us in collecting and maintaining this personally identifiable information. However, we require such service providers to maintain the confidentiality of such information.
For security purposes, we do not keep your credit or payment card data in its entirety. We pass all credit and payment card data through to our payment card processor, TrustCommerce, and maintain only the last 4 digits of your card number, together with the expiration data, your name, your address and the secure transaction identification number for your transaction issued by Verisign in order to help track and confirm any payment you make to us. As a result of TrustCommerce’s services to us, TrustCommerce will capture data necessary to provide the payment processing services requested by Advocate Aurora Health. Unless otherwise prohibited by applicable law, TrustCommerce may use the data as necessary to (i) perform services for Advocate Aurora Health, (ii) collect and process data for record keeping, reporting, and analytics, (iii) provide fraud detection, risk modeling, and support services, (iv) compile and disclose data in the aggregate such that an individual is not identifiable, including calculating average transactions by region or industry, and (v) provide data as required by the card associations, law or court order, for purposes of incident investigation, or to defend TrustCommerce’s rights in a legal dispute. In addition, in order to provide you with certain features, our Application may request information from servers and systems owned or operated by Advocate Aurora Health or its third-party vendors, and those servers and systems may record technical information about that request such as an IP address. Our Application will also request one or more times your permission to turn on Location-Based Services, enabling you to better locate care facilities, specific services available based on your location and doctors near you and more easily arrange transportation to a care facility, should you choose to access this service.
Below is information around additional data we collect and how it is handled:
- Location: LiveWell may use your location data to notify front desk staff when you've arrived for an appointment or to suggest healthcare providers that are near you. LiveWell will not store your location data.
- Camera: LiveWell may use your camera to take new photos or to capture video for video visits. The photos you take may be used to personalize your account or used as file attachments that are sent. LiveWell may store data collected from the camera in your medical record.
- Microphone: LiveWell may use your microphone to capture audio for video visits. LiveWell will not store your audio data.
- Storage: LiveWell may access your device's storage to read and write files you choose to use in the application. These files may be used as file attachments that are sent to your provider or they may be created from file attachments sent to you from your provider. LiveWell may store files uploaded from your device's storage in your medical record.
- Bluetooth: LiveWell may access your device's Bluetooth. Bluetooth will be used to detect other nearby devices to help with appointment arrival. LiveWell will not store your Bluetooth data.
- Phone: LiveWell may use your phone to call phone numbers displayed in the application. LiveWell will not store your call history or other call data.
How we use the non-personally identifiable information we collect
In addition to those uses set forth above, we use non-personally identifiable information in the aggregate to determine how much traffic the Website receives, to statistically analyze Website usage, to improve our content, and to customize the Website’s content, layout and services. In addition, we may use your IP address to help diagnose problems with our server, to manage the Website and to enhance the Website based on the usage pattern data we receive. Please note that information you enter into general search bars is considered non-personally identifiable information and may be stored and used in the aggregate as provided above.
Retargeting and remarketing
We have also enabled additional Google Analytics Advertising Features: Google Display Network Impression Reporting and Remarketing with Google Analytics. Handling of electronic records & backup
In general, we will retain all information collected through the Website for, at a minimum, the length of time permitted by law. However, we will delete any personally identifiable information in our database upon your request or as otherwise required by law. Generally, information collected through the Website will be destroyed as soon as is reasonably practicable following the expiration of the applicable retention period, taking into consideration that on certain electronic media and software platforms there is not currently the technical ability to “delete” or “destroy” a record. Attempts will be made to notify you prior to the destruction of your medical records only as required by law. Destruction methods will be used that ensure the records, whether paper or electronic, are rendered essentially unreadable, indecipherable, and otherwise not capable of being reconstructed. We maintain backup files as a protection against natural disasters, equipment failures, or other disruptions. Backup files protect you and us because they lower the risk of losing valuable data. Backup files may contain records with your personal information. Removing a record from our active files and databases does not remove that record from any backup systems. Such backup data will eventually be passively deleted as backup records are erased through the normal recycling of backup files.
