Make a gift
When you make a gift in honor or in memory of someone, we'll send a personalized notification of your thoughtfulness to whoever you want to notify (if contact information is provided). Please note that we will not disclose your gift amount.
- Donate now. Please be sure to note the name of the person you are honoring and include the contact information of the person(s) you would like us to notify.
- Give by mail. Send in your check with one of the forms below.
Request memorial gifts
We understands the emotional period following the loss of a loved one. Many families take comfort in requesting donations to a hospital or program as a meaningful way to remember a loved one. Families may also invite others to give by using the following sample language in the obituary:
In lieu of flowers, the family requests donations be made to <Advocate or Aurora hospital or site name> for <area of support>. Please send memorial gifts to <Advocate Charitable Foundation or Aurora Health Care Foundation>, 2025 Windsor Dr., Oak Brook, IL 60523, or give online at aah.org/foundations.
Please notify us if you will have memorial gifts designated to our hospitals or programs.
